The Extended Day Service Program at Our Lady of Lourdes School will begin the first full day of school.

 

In order for your child to be enrolled in the After Care Program for the 2008/09 school year, the following is required:

 

1) Completed registration form with a nonrefundable annual $25 registration fee per child

2) Completed and notarized release form

3) Payment for one monthin advance, due the first school day of each month a schedule is detailed below

 

All the above forms and fees must be received by the Business Office no later than August 18, 2008. If any item is missing or if monthly payments are not made on a timely basis, your child will not be eligible to participate in the program.
